Job Description:
Duties and responsibilities of this position include but not limited to the following task and assignments.
The Social Worker will participate in resident management.
Focus on admissions and coordinate the process of discharge planning.
Follow up on discharges.
Responsible for the coordination of services for example; home care, equipment transportation, dialysis and assisting residents and their responsible parties in locating community resources.
Assist in obtaining resources from community social, health and welfare agencies to meet the need of the resident.
Evaluate social, and family information and assist in determining plans for social treatment.
Participate in resident care plan for long term care and skilled residents.
Applicable placements when needed.
Attend meetings as necessary.
Assist with coordination of room changes.
Focus on customer service.
Maintain quality relationship with the medical profession.
Knowledge of MDS is required.
Possesses a Masters/Bachelor's Degree, preferably in social work, and experience in a long term and skilled nursing homeJob Type:
